Mythic Uniques
The classic Mythics (Harlequin Crest, Tyrael's Might, etc.) remain strictly Mythic quality only (the PTR trial of lower non-mythic unique versions was reverted based on player feedback). Every lair boss and Prime Evil can drop them on Torment difficulty, with Belial, Lord of Lies and the Prime Evils offering elevated odds.
Boss Trophies → Horadric Cube
Lair boss hoards drop Boss Trophies. Cube one to craft a dedicated unique from that boss — and there's a small chance it comes out Mythic. Crafted items carry the Crafted tag.
The General Unique Pool also rides along on every lair boss kill, though since Lord of Hatred it is a much smaller slice of boss loot. Each item's own card says whether it is in that pool.
Season 15: Prime Evils & Splinters
The three Prime Evils in Waking Nightmare capstone dungeons (Torment 10+) share an elevated Mythic Unique drop table and drop Splinters of Evil:
- Diablo (Tristram Cathedral) → Splinter of Terror
- Baal (Arreat Summit) → Splinter of Destruction
- Mephisto (Durance of Hate) → Splinter of Hatred
Cube all 3 splinters in the Horadric Cube to craft Wirt's Lucky Coin. Toss the coin into the Old Tristram fountain to trigger the Pandemonium Event (fighting all 3 Uber Prime Evils) for the Hellfire Torch (Mythic Unique Charm).
